Submit your Grievance form


In case you have certain Grievance regarding your pension against the bank or the Disbursing authority, you can fill and submit the Grievance form and the CPAO will look after your complaint.

Life certificate customer care at Jeevan Pramaan and CPAO for pensioners



Following are the types/subjects of Grievances to be addressed in the form. Check them out if you have any of these:
  • Authorized at wrong bank.
  • CVP /Additional CVP/Addl DCRG not paid.
  • Check accuracy of revised pension/arrears.
  • Commuted portion of pension not restored after 15 years.
  • DR arrear payment delayed.
  • Delay in First Credit.
  • Delay in preparing duplicate PPO.
  • Delay in processing New/Revision case.
  • Details of overpaid pension not provided.
  • Due and drawn statement of arrear not provided by bank.
  • Family pension not started to spouse /children after death of pensioner, both halves not returned.
  • Fixed medical allowance not Paid.
  • Form - 16 not provided by bank.
  • Include name of spouse/DOB/Address etc.
  • Incorrect revision by department.
  • Less/Excess pension paid by bank.
  • Life time arrear (LTA) payment delayed.
  • Mistake  in name/address/date of birth etc.
  • Others (specify reasons eg case not received from PAO),Mistakes by postal deptt.
  • PPO book not handed over to Pensioner.
  • Payment of fixed medical allowance not stopped.
  • Pension account not transferred to other bank.
  • Pension stopped even after giving life certificate.
  • Pension/Family Pension/Additional pension not paid.
  • Revised pension/Arrears not paid.
  • Speed Post sent at wrong destination.
  • Want a copy of SSA.
  • Wrong bank BSR Code/Wrong account number.
  • Wrong deduction of Income Tax by bank.

Apart from submitting your Grievance, there are two other things you can do at the Grievance page: 1) Check the status of your complaint and 2) Search the utility by submitting your entering the keywords like your name, ppo number, account number etc.

Although the Life certificate is submitted instantly and there is no need to wait and check for the submit status. The pensioner will just download their DLC and it is the proof that the certificate has been submitted. Even after doing all of this if there arises an issue you can contact them at the above given address, phone number or E-mail address.
